Admission Procedure for Prospect Students
 

APPLICATION:

Complete and return the application form along with a non-refundable $50.00 evaluation fee. If the student is admitted to Be Unique Academy Prep, the $50.00 evaluation fee will be deducted from the registration fee. In addition, the following documents must be submitted:

 

  • Copy of last IEP (Individualized Education Plan)

  • Copy of Behavior Intervention Plan, if applicable

  • Copy of most recent psychological evaluation

  • Copy of last year’s report card

  • Copy of most recent standardized test score, if applicable

 

ASSESSMENT:

Once we have received your completed application, you will be contacted to set up an appointment. The assessment requires your child to spend a full day at the school. The school director along with the evaluation team will evaluate your child’s academic, fine and gross motor, social, and behavioral skills, to ensure appropriate placement. Depending on your child’s literacy level, your child will also be assessed through the Basic Reading Inventory, which is a statewide approved literacy-testing tool. You will be notified in writing within 10 business days from the date of evaluation if your child has been accepted to the school. 

 

REGISTRATION:

Once notified that your child has been accepted to Be Unique Academy Prep, parents have 14 business days from written notification to return the enclosed registration packet with the registration fee (Registration Fee $250 - Evaluation fee $50 = $200). 

 

Admission requirements:

  • Students with mild to high developmental and intellectual disabilities

  • Students must demonstrate independence in self-care skills in toileting and eating

  • Students who display non-aggressive behaviors to self or others

  • Students entering Kindergarten through High School

  • Students must be able to function in a classroom setting with a ratio of 4:1
